Home Visit Programme

We don't always have beds available in the Respite Unit for patients in need, and so patients are placed on a waiting list and a nurse conducts a home visit to assess the patient's situation, provide home based care where possible, and ascertain how critical and time-sensitive their admission to the respite unit is. When patients are discharged from the Respite Unit our nurses and care-givers visit them in their home a number of times, to ensure that their health continues to recover; to conduct pill counts and provide adherance counselling; and to provide ongoing acceptance and disclosure counselling.
